Judging from the carapace and tentacles that the leech is forming, I’d say Zak’s nihillium is commandeering the nihillium in the leech. Probably the result of an auto-repair mechanism that enables a Nihallak to automatically regenerate when injured, and powered by the leech’s High Plane energy it has accumulated. The bite of Zak the leech took doesn’t realize that it’s not the original. More interesting is that the new creature has some of Zak’s mental capacity and memories.

So it’s either a Metamorphic creature, or some form of Prot-Nihallak.
